1. COMPOSITION
Each film-coated tablet contains:
Bisoprolol Fumarate IP
Equivalent to Bisoprolol 1.25 mg
Excipients: q.s.
2. INDICATIONS
BISOHAB 1.25 is indicated for:
Chronic heart failure with reduced ejection fraction (HFrEF)
Hypertension
Chronic stable angina pectoris
3. DOSAGE AND ADMINISTRATION
Chronic Heart Failure
Bisoprolol treatment should be initiated at a low dose and gradually increased under medical supervision.
A commonly used starting dose is 1.25 mg once daily, particularly during initiation of therapy for chronic heart failure.
The dose may be gradually increased according to individual tolerance and clinical response, generally at intervals of at least 2 weeks, up to the maximum tolerated/recommended dose.
Hypertension and Angina
The usual starting dose is generally 5 mg once daily. Lower doses may be appropriate in selected patients.
BISOHAB 1.25 should be taken once daily, preferably at the same time each day, and may be taken with or without food.
The tablet should be swallowed with water and should not be stopped abruptly without medical advice.
4. CONTRAINDICATIONS
Bisoprolol is contraindicated in patients with:
Hypersensitivity to bisoprolol or any component of the formulation.
Acute heart failure or during episodes of decompensated heart failure requiring intravenous inotropic therapy.
Cardiogenic shock.
Second- or third-degree atrioventricular block in patients without a functioning pacemaker.
Sick sinus syndrome or sinoatrial block.
Severe bradycardia.
Severe hypotension.
Severe peripheral arterial occlusive disease.
Severe bronchial asthma or severe chronic obstructive pulmonary disease, depending on clinical assessment.
5. WARNINGS AND PRECAUTIONS
Use bisoprolol with caution in patients with:
Bronchospastic disease such as asthma or COPD.
Diabetes mellitus, as beta-blockers may mask symptoms of hypoglycaemia.
Peripheral vascular disease.
Thyrotoxicosis, as beta-blockers may mask symptoms.
Prinzmetal's angina.
Renal or hepatic impairment.
A history of allergic reactions or those receiving desensitization therapy.
Do not discontinue bisoprolol abruptly, particularly in patients with coronary artery disease, as this may cause worsening of angina or other cardiac complications.
6. ADVERSE EFFECTS
Common adverse effects may include:
Fatigue
Dizziness
Headache
Bradycardia
Hypotension
Cold extremities
Nausea
Vomiting
Diarrhoea or constipation
Other adverse effects may include sleep disturbances, depression, bronchospasm and worsening of heart failure, particularly during dose initiation or titration.
7. DRUG INTERACTIONS
Caution is required when bisoprolol is used with:
Other medicines that lower blood pressure or heart rate.
Calcium-channel blockers such as verapamil or diltiazem.
Antiarrhythmic medicines.
Digitalis/digoxin.
Insulin and oral antidiabetic medicines.
Certain medicines affecting hepatic metabolism.
The treating physician should review all concomitant medications before starting treatment.
8. USE IN PREGNANCY AND LACTATION
Bisoprolol should be used during pregnancy or breastfeeding only when clearly indicated and under medical supervision.
9. OVERDOSAGE
Overdose may cause marked bradycardia, hypotension, bronchospasm, hypoglycaemia and acute cardiac complications.
Suspected overdose requires immediate medical evaluation and appropriate supportive treatment.
10. STORAGE
Store below 30°C.
Protect from light and moisture.
Keep out of reach and sight of children.
Store in the original package.
